Preventing Falls Among The Elderly Population - 1390 Words

Introduction Falls are a commonly experienced problem among the elderly population. The incidence rates of falls among the Hong Kong’s community-living elderly stood at 26 percent, while the incidence of new fallers stands at 198 falls per 1000 individuals per year. Whereas, one-year prevalence level of falls was identified to be 19 percent with the mean number of falls annually for a faller being 1.4 falls. Forty-seven of the falls in a year were identified occur indoors while the rest outdoors. Around a third of the elderly population aged over 64 years experience at least one fall a year. Past studies link falls to extrinsic factors and intrinsic factors. An example of intrinsic factors includes medical problems while one extrinsic factor involves environmental hazards. Comprehensively, falls result from muscle weaknesses, gait deficits, usage of assistive devices, cognitive impairments, arthritis, advanced age and history of falls (Fong, Siu, Yeung, Cheung, Chan, 2011). This paper proposes a research study focuses on preventing falls among the elderly population in Hong Kong. The target population of the study constitutes the elderly aged over 60 years on the hectic area named Mong-Kok, Kowloon in Hong Kong. Consider a man that looks to material needs as the necessities of life. He moves through his world in a twenty-four hour cycle of the mundane, never reaching for a less ignorant existence. Bertrand Russell believes that these â€Å"practical men†, as society deems them, are wrongly named. The value of philosophy can be found when anyone chooses to step over the line between things and ideas. I am claiming, in this instance, that philosophy is valuable for being a source of knowledge and understanding, among other things. Those that attempt to gain these are in turn going to benefit from their efforts. A man does not necessarily need the ability to comprehend the entire universe, but just to be open to thought. In the past, men that worked towards this task of thinking, such as Newton, were able to take philosophy and evolve it into a separate science. This reasons that philosophy’s value is largely in the possibility of a greater enlightenment that has yet to be determined. There is value in the fact that a deeper reality exists. That life does not just run blindly through time, but streams around reason and thought. Knowledge should alone be enough of a value for philosophy to be an appreciated source of gaining exactly that end. Thomas Nagel writes, â€Å"†¦humans have the special capacity to step back and survey themselves, and the lives to which they are committed†¦Ã¢â‚¬  (page 23). This realization is one of the reasons that philosophy contains value for the society at large. Everyone, through examining and doubting their choices, can gain knowledge. And knowledge is the primary aim of philosophy, according to Russell and my own opinion. Socrates summarizes it best in Plato’s, Apology: Defense of Socrates, when he stated, â€Å"†¦an unexamined life is no life for a human being to live†¦Ã¢â‚¬  (page 40). Humans were given the capacity to have thought processes and go beyond the routine existence of lower level life forms. To let this possession go unused would be neglecting the possibilities of the mind. However, the value of philosophy for society at large is limited by self-assertion. The masses will find themselves looking for knowledge but being blocked by the view that the world is of less worth than themselves, or the Self. This will be the downfall of the instinctive man; he is contained in his private interests. It is almost like a trap, man fills his life with family and friends and believes that he has found his place in life. A true student of philosophy will have a want of knowledge that is free and pure. This want contains no concerns of Self, but rather of the not-Self. Knowledge arrives when man lets go of trying to fit the universe into his world and instead fits his world into the universe. In order to be a philosopher, one must overcome the narrow circle of the Self and of private interests. Therefore the largest value of philosophy is for the philosopher, for he is able to completely be open to the acquisition of knowledge. Most of the value of philosophy is then sent indirectly to the larger society. The fact that philosophy, as a subject, is prone to uncertainty can arouse disbelief in its value. It can be argued that no knowledge can possibly be gained by studying a field in which there are no definite answers. Russell agrees with this point when he maintains that even if answers are determined, none of them can be proven true without exception. The subjective areas of thought, those dealing with opinions and differences in beliefs and practices, would hold no basis in practicality. â€Å"Practical man† will continue to waste away in his secluded reality, convinced that being materialistic is the most important quality to possess. Russell himself mentions, â€Å"†¦many men, under the influence of science or practical affairs, are inclined to doubt whether philosophy is nything better than innocent but useless trifling, †¦and controversies on matters concerning which knowledge is impossible† (page 9). However, Russell contradicts his own statements on purpose with the idea that people have the wrong view of philosophy. The uncertainty in philosophy is what makes the subject intriguing and worth arguing for or against. So what if there are no definite answers? The process of coming to the conclusion that nothing is set in stone is where the knowledge lies in wait to be learned. The questions of life make for intellectual freedom in the search for the unfound answers. Philosophic contemplation best works when the desire for knowledge is unadulterated. This would then deal mainly with the area of the not-Self; it must be in union with the Self to create the right environment for the intellect. Russell helps to confirm my statement that knowledge is the value of philosophy when he writes, â€Å"†¦free intellect will see†¦without traditional prejudices†¦in the sole and exclusive desire of knowledge-knowledge as impersonal, as purely contemplative, as it is possible for man to attain† (page 11). J. J. C. Smart believes that we should never assume that we have found the ultimate and final truth about anything. But that having a condensed view will bring us closer than not pondering it at all. Then, any attempt to push beyond that line between things and thought will create a positive end. Knowledge, the total range of what has been perceived and learned, is the absolute value of philosophy in my opinion. Philosophers, as well as man, can only benefit from the scrutiny placed on thought. Without the knowledge that philosophy can provide, the world would be a very simple place based exclusively on materialistic views. The old saying that ignorance is bliss would unmistakably be true. Man would continue in his everyday life, unaware of the chance that he is missing. IV. Choice and Rationale After reviewing all of the possible choices I chose to go with choice A. I chose A, because it saves Pacific Healthcare a lot of money as discussed in choice C. By choosing DuPont the company would save $450 a day compared to Kodak. That is $3,150 per week of savings and $12,600 a month by switching to DuPont. Also in choosing Agfa which is the same quality as Kodak, Pacific Healthcare would save $330. Keep in mind that Agfa and DuPont have the same quality as Kodak. Why choose Agfa over DuPont if they have the same quality of X ray film? That is why I chose DuPont. It is a smart solution, because it saves Pacific Healthcare money while also keeping the same quality as Kodak. Moving on two the other choices Mr. Rubble has. In the beginning I mentioned that there were four competitors that offered X ray films at a much lower rate than Kodak. The competitors were Agfa, DuPont, Fugi, and 3M. I have discussed Agfa and DuPont. The reason for not adding Fugi and 3M to my possible solutions is because Pacific Health care wanted to keep the same high quality in X ray film. Even though Fugi and 3M met minimum requirement they were thought of as having lower quality. Pacific Healthcare was looking for obtaining the highest quality with the best service at the lowest overall cost. To me they did not meet the requirement of Pacific HealthCare’s goal. V. Implementation I would let Pacific Healthcare know of my overall decision. In this brief, I would brief all my possible choices I had and why I chose to go with my choice. If we are in agreement my next step would be to contact DuPont and request their service for X ray film. From there I would let Kodak know that we appreciate their business, but we no longer are in need of their X ray film service. Of course I would make sure that DuPont has agree to be our single source supplier for X ray film prior to finishing the contract with Kodak. Appendix A What alternatives should Barney Rubbles consider when addressing the problem? Barry Rubbles should consider the high quality that Pacific Healthcare would like to keep as well as the best service with overall lower cost. He should consider which one would give the overall high quality as well as trying to save money. Should Pacific’s’ supply policies allow for any medical staff personnel to control sourcing decisions? In my personal opinion no, because they may not have the proper training or know what the companies needs are. I would like the professionals handle that. This case does not show a good example, because Mr. Howell chose the highest competitor. I would definitely have had both Mr. Howell and Mr. Rubbles work together. Sometimes one may come up with a different solution. What are the advantages and disadvantages of staying with Kodak—or changing suppliers? How would you evaluate these? If the Pacific Healthcare would stay with Kodak they could keep receiving their discount for the other services that Kodak provides. This includes the X ray equipment, maintenance and, service for being their only source for X ray film. Like previously stated, I am not sure exactly how much that discount was. Nor did I know how much the competitors were willing to give those services to Pacific Healthcare. With changing suppliers Pacific Healthcare would definitely save a lot of money. This would include changing to DuPont or Agfa. With DuPont the company would save $450 a day and by switching to Agfa the company would save $330 a day. Mr. Rubble has to keep in mind that both have the same quality of X ray film as Kodak. What action could Mr. Rubble have taken prior to Mr. Howell’s death to obtain reduced film prices? Mr. Rubble should have conducted a research just like he did when they put him in charge of the X ray film ordering. He should have presented this information to Mr. Howell and if he had disagreed Mr. Rubble should have taken it up to the company.
